VOUCHERS The Golden Arrow Center for Mothers and Children began a new system for usage of the baby closet by all clients beginning April I, 2003. As before, clients may receive emergency food and formula once per month and every two months may recieve clothing and diapers. INSTRUCTIONS FOR VOUCHER
SHEET: 1.) Clients may receive a "full" baby closet visit, which includes maternity clothing, baby clothes and other necessities, after they have accomplished four tasks pertinent to their own physical and emotion health or education needs. Suggested "tasks" or referrals are listed on the following sheet. The client is asked to bring this form to the agency they have been referred. (Please ask a representative of that agency to stamp, sign, and date your form next to their address and phone number) The client should bring their completed voucher form to The Golden Arrow Center for Mothers and Children after they have achieved (4) referral visits of which Two must be from the Golden Arrow's In-House Classes. *Besides receiving 2 of your 4 referral visit signatures, you will receive extra diapers for attending our in-house classes. You may choose to do all 4 task at The Golden Arrow’s in house classes. No hand written signatures: They must be on letterhead or an address stamp that is signed and dated! 2.) You may also volunteer (18) hours at another agency or (18) hours at your child's school. Please have the director, case manager, secretary, or principal sign on their letterhead and place the number of hours volunteered. 3.) As before, if you are working outside the house (full time or part time), you may demonstrate the last three (3) consecutive pay stubs. Please bring in these pay stubs, when you return for clothing. 4.) You may obtain a full time student statement from your school for every two months.The Golden Arrow Center for Mothers
